Rose Meadow Farm is a unique, assisted living facility for people with acquired brain injury (including neurological injuries such as stroke) and/or spinal cord injury. Our mission at Rose Meadow Farm is to provide each resident with the dignity and self-respect that we all expect to receive, and to assist each resident individually to attain the highest quality of life possible.

We serve people who are...

diagnosed with an Acquired Brain Injury and/or spinal cord injury, CVA, Anoxia (or other related neurological trauma).
medically stable
ventilation dependent
requiring total care which may include tube feedings and trach care
requiring up to 24 hours of supervision and structure for personal safety and well being.
in need of nursing support, medical management and/or attendant care
needing support with activities of daily living, planning, money management, personal care, mobility and transfer, bowel and bladder management, community reintegration, cognitive training, and safety.

Our staff has the ability to address a wide range of individual needs- from the very dependent individual who might need lifelong care to the individual who needs short-term rehabilitation to transition back into the community.
